What is subcision with injectables? Subcision is a minimally invasive procedure used for selected depressed or rolling acne scars tethered by fibrous bands beneath the skin. A specialised needle or cannula is passed below the scar to release the tethering. An injectable may be used where clinically appropriate, but it is not required in every case. What may it help with? Subcision is mainly considered for suitable rolling or tethered atrophic scars. It is generally less effective for narrow ice-pick scars, raised scars or active inflammatory acne. Scar treatment can improve appearance but cannot remove every scar completely. How does it work? Releasing the fibrous bands allows the depressed area to lift and initiates a healing response. A selected hydrating or biostimulating injectable may provide structural support or support gradual collagen remodelling. Injectables cannot guarantee that a scar will not re-tether. Suitability A consultation is required to assess scar type, active acne, skin type, medical history, medicines, previous treatments and tendency to pigment or form raised scars. Active infection or uncontrolled acne may need treatment first. Procedure and recovery The area is cleansed and an agreed anaesthetic is used. Bruising, swelling, tenderness, pinpoint bleeding, firmness and temporary numbness are common. Recovery varies with the number and depth of scars and whether an injectable is used. Risks and limitations Risks include bleeding, haematoma, infection, prolonged swelling, pigment change, lumps or nodules, altered sensation, unevenness, scarring and an incomplete or temporary improvement. Injectable-specific risks include inflammatory reactions and rare vascular complications. Further sessions or another scar treatment may be recommended. Aftercare Follow the clinic’s cleansing, skincare and activity instructions. Avoid rubbing, picking and strong active products until advised, and use daily broad-spectrum SPF. Contact the clinic urgently for severe or increasing pain, blanching or unusual skin colour, spreading redness, marked swelling, visual symptoms or unexpected discharge.
